Activity Description and Educational Objectives

In this activity, a cardiology expert discusses clinical guidelines and practical recommendations in the treatment and management of heart failure for family physicians. After completing this activity, family physicians will be better able to identify and implement strategies for the multidisciplinary care of heart failure patients and discuss self-monitoring and management with patients.

Upon completion of this activity, participants should be better able to:
  • Implement evidence-based guidelines for early recognition and diagnosis of heart failure
  • Apply available heart failure therapy based on ACC/AHA guidelines and clinical efficacy and safety data
  • Identify strategies to improve multidisciplinary coordination of heart failure care and promote patient self-monitoring and management of disease symptoms

Target Audience

This activity has been designed to meet the educational needs of family physicians and other clinicians involved in heart failure.

Requirements for Successful Completion

In order to receive credit, participants must view the activity and complete the post-test and evaluation form. A score of 70% or higher is needed to obtain CME credit. There are no pre-requisites and there is no fee to participate in this activity or to receive CME credit. Statements of Credit are awarded upon successful completion of the post-test and evaluation form.
