Activity Description and Educational Objectives

In this activity, experts in managing patients with chronic heart failure (CHF) discuss optimization of therapy and collaboration with a multidisciplinary team of healthcare professionals to address changes in functional status and adverse events that may arise during treatment. In addition, the expert panel provides an overview of updated CHF treatment guidelines, as well as the latest real-world evidence on cost-effectiveness and long-term efficacy and safety of new therapies.

Upon completion of this activity, participants should be better able to:

  • Employ heart failure pharmacologic and non-pharmacologic strategies according to current guidelines and individual patient profiles
  • Apply key clinical trials of newer heart failure therapy to improve patient quality of life and reduce hospital readmissions and healthcare costs
  • Identify evidence-based strategies to improve the longitudinal care of patients with heart failure
